
To open a credential file, you will need to complete the basic forms (below), and you will need to pay the initial registration fee.
Fee for current students: $20.00 to open a credentials file. Fee for alumni: $25.00 to open a credentials file.
The registration fee must accompany your registration materials. Please make checks payable to “Boston University.”
The following PDF forms can be printed and mailed in, and are also available by stopping in to our office at 19 Deerfield Street.

Consent Form This form MUST be completed before any of your credentials can be sent.
Basic Reference Form and Evaluation Form
THE FAMILY EDUCATION RIGHTS AND PRIVACY ACT OF 1974 (Public Law 93-380) provides that registrants have the right to read references placed in their files after January 1, 1975, UNLESS they have waived this right.
IF THE REGISTRANT WAIVES THE RIGHT OF ACCESS TO A REFERENCE, IT IS CONFIDENTIAL AND ITS CONTENTS WILL NOT BE MADE KNOWN TO THE REGISTRANT. In the case of a discrepancy between the consent form and the individual reference form, the designation on the reference form will take precedence.
Although we cannot give you access to the contents of confidential files, we are happy to let you know what documents have been received and are active in your file.
Before giving out reference forms to those who will write your reference letters:
- Type or print your first, middle, and last name (legal name only).
- Read the waiver information and check the appropriate box.
- Sign and date.
- Original references sent to this office are the property of Boston University, and only copies of these documents will be forwarded.
- You may call the Credentials Office, 617-353-3590, Monday through Friday, between the hours of 9:00 and 5:00 if you need further information.
- Before you give out your reference forms, you must first open a credential file. It is impossible for us to keep references for non-registered students.

Official transcripts may only be supplied by the Registrar's Office. You may insert an official transcript into your file that will be photocopied along with your other documents. When sent in
this manner, they are considered UNOFFICIAL TRANSCRIPTS. If you wish the Credentials Office to send OFFICIAL TRANSCRIPTS with your references, you must supply the Credentials Office with as many OFFICIAL COPIES
as you need for each mailing. The Registrar's address is 881 Commonwealth Avenue and the phone number for transcripts is 617-353-3616.

Career Services strongly believes that a resume or CV, together with a cover letter, should be sent separately from all other documents. It is not recommended that you use the Credentials Service as a means for mailing out resumes or CV's to prospective employers, as these documents could change frequently. However, you may keep a copy of these items in your file should the unusual circumstance occur where you are requested to submit a copy of your resume/CV with your references.
